JOB PROGRAM: Permanent Supportive Housing (PSH) Residential, Youth & Emergency Services (RYES)
QUALIFICATIONS:
Bachelor’s degree in human services field or equivalent related experience. Two years experience in human services related area. Experience working with youth ages 16-24 preferred. Require own transportation, valid driver’s license and up-to-date insurance. Excellent interpersonal skills and good organizational skills. Experienced in working with computers; knowledge of Windows environment preferred.
JOB RESPONSIBILITIES:
The Case Manager is responsible for assessing the needs of the youth housed through the Division of Housing’s Permanent Supportive Housing project. Works with treatment providers and others to assist the youth, who are HUD defined chronically homeless, in stabilizing and maintaining stability in their unit. Assists them in developing goals, obtaining income, setting financial plans and reuniting with family in a manner that is Housing First. Links youth to community resources to assist them in exiting homelessness permanently by assisting them to remain in housing. The position requires ongoing interaction with youth, staff, volunteers, housing provider and others in the community.
SALARY: $15.71 per hour
HOURS: Monday – Friday 8:00 a.m. – 5:00 p.m.
